Who goes first in Egyptian Rat Screw?
Bicycle Cards rules, standard 52-card pack (English)
The player to the left of the dealer plays the first card, then play continues around the table.

Rule details
Anyone may slap a double or sandwich, so the first turn gives little advantage.
